All QOP Volunteers: Lectors, EMs, Ushers, Altar Servers, Sacristans, etc. need to complete a Volunteer Verification Form. Click: Volunteer Verification Form – Formulario de Verificación de Voluntario.

And any volunteer over the age of 18 needs to have a background check (renewal needed every six years) and Safe Environment Training (renewal required EVERY year) completed before serving in the Diocese of Phoenix. Please go to phoenix.cmgconnect.org. If you need help, please reach out to Maureen Battles (English speaking) or Rudy Navarro (Spanish speaking).

For children who volunteer at QOP who are under 18, they need to have a completed Youth Safe Environment Form. Parents, please fill out this form: English SET Youth Form.
