Even if you have already registered, please complete the form again.  You can deliver it to us in several methods:

1. In Person: by leaving it in the parish front office
2. By Snail Mail: to 141N Macdonald, Mesa, AZ, 85201
3. Offertory Basket: just place your completed form in the offertory basket next time to attend mass.
